Lima Memorial Receives 5-Star Rating for Home Health Care

Lima Memorial Home Health Care is pleased to announce it has received a 5-star rating, the highest rating achievable, in patient satisfaction from the Center for Medicare and Medicaid Services (CMS). The star rating, instituted by CMS as part of its Home Health Compare, is a tool consumers can use when choosing a home health care provider. This prestigious rating indicates our patients and their families believe we provide exceptional care and a high-quality patient experience.

Lima Memorial Home Health Care works with patients, families, caregivers and physicians to help people maintain their independence at home after being discharged from the hospital or a long-term care facility. Our experienced home care staff, including registered nurses, therapists and trained home health aides, travels to individual homes throughout a seven-county area. This includes Allen, Auglaize, Hardin, Logan, Mercer, Putnam, Van Wert County and beyond. Our team teaches patients and families how to care for their specific health needs through continued care, education, treatment, intervention and monitoring in the patients’ own home.

  • Produce comparable data on the patient’s perspective that allows objective and meaningful comparisons
  • Quality public reporting of survey results that create incentives for agencies to improve their quality of care
  • Quality public reporting that enhances public accountability in health care by increasing the transparency of the quality of care provided in return for public investment

The HHCAHPS comes with a list of requirements and guidelines that help aid the public in accurately rating the home health care facilities.
