The Community’s Voice at Lima Memorial
The Lima Memorial Patient and Family Advisory Council (PFAC) is a partnership of patients, family and staff advisors. The council works diligently to promote the concept of patient-centered care to enhance the delivery of health care at Lima Memorial Health System.
Advisors help:
• Promote patient-centered care
• Advise on relevant hospital policies and procedures
• Make recommendations to provide input for patient services and needs
Who is on the Advisory Council?
A patient and family representative on the Advisory Council is any person or the immediate family member / guardian of a person who has been hospitalized at Lima Memorial Health System within the last three years.
Advisors will:
• Provide information to staff about the needs and concerns of patients, caregivers and family members.
• Advise on relevant hospital policies and procedures
• Collaborate with staff to improve services and programs that affect patients and family members.
• Offer insight and recommendations for improving quality, safety, access, education and patient & family satisfaction.
