We will prepare your paperwork in advance. We recognize the confidential nature of the information you submit online. Services You Can Pre-Register For Currently, you can pre-register for the following departments and procedures at Lima Memorial Health System: Mammogram Radiology (X-ray, CT or MRI) Laboratory Hospital inpatient and outpatient surgery Scheduled outpatient procedures (GI Lab, peripheral vascular) What You Will Need to Pre-Register Your insurance ID card information Social Security Number & Date of Birth of the card holder Procedure date and time Type of procedure (such as a mammogram or X-ray) Personal contact information Physician order After You Have Finished Online Pre-Registration After you have submitted your online pre-registration form, you will receive a confirmation e-mail from Lima Memorial. You will also be reminded to print your completed form and bring it with you along with your doctors order and insurance cards on the day of your procedure, along with any items listed on your pre-registration form.
