Stay a Little Longer to Get a Little Stronger
Our Transitional Care Center (TCC) serves as a transition in your care following surgery, injury or acute illness. TCC will teach you new ways to care for yourself, improving your current abilities, completing treatment and ultimately enabling you to leave the hospital setting.
During your first few days on TCC a care plan will be developed for you, and during your stay meetings will be conducted to review your plan. Family conferences will also be conducted to allow your loved ones to ask questions, voice concerns and assure your comfort and overall understanding of the program.
Caring about your recovery goes beyond your stay. After you are discharged, you will receive a telephone call to assure you have received all the equipment and services that were ordered prior to discharge.
Family is Part of Your Team
Your family or significant other will be part of your daily routine by providing moral support and participating in therapy sessions. Because of this, all visiting hours are open, but therapy takes precedence.
