Our Hospice Care Partnership
Lima Memorial offers inpatient hospice care in partnership with
Putnam County Hospice. Hospital staff and hospice
professionals work together during our patients' stay to provide
the best care possible. Hospital staff members are here to ensure
that our patients' primary care needs are met, while the
hospice nurse will visit daily to help reduce symptoms and provide
support.

our patients' and their family aware of all discharge

Putnam County Hospice was established
in 1996 with the dream of allowing our residents to spend their
last days at home among the people and things that they love.
Hospice recognizes the unique and individual needs of the
terminally ill patient and their family.
Our goal is to provide comfortable care, fostering an atmosphere
in which an individual can retain personal freedom and the ability
to make independent decisions. Hospice affirms life and neither
hastens death nor prolongs life. It allows one to live each day
