Lima Memorial Health System's online patient pre-registration process is a handy, timesaving option that allows you to pre-register at your convenience.

We will prepare your paperwork in advance. We recognize the confidential nature of the information you submit online. For this reason, you will be transferred to a secure page to complete your pre-registration.

Services You Can Pre-Register For
Currently, you can pre-register for the following departments and procedures at Lima Memorial Health System:

  • Mammogram
  • Radiology (X-ray, CT or MRI)
  • Laboratory
  • Hospital inpatient and outpatient surgery
  • Scheduled outpatient procedures (GI Lab, peripheral vascular)

What You Will Need to Pre-Register

  • Your insurance ID card information
  • Social Security Number & Date of Birth of the card holder
  • Procedure date and time
  • Type of procedure (such as a mammogram or X-ray)
  • Personal contact information
  • Physician order

After You Have Finished Online Pre-Registration

After you have submitted your online pre-registration form, you will be reminded to print your completed form and bring it with you along with your doctors order and insurance cards on the day of your procedure, along with any items listed on your pre-registration form. You must pre-register at least 24-hours before your appointment.
